A sudden change in health can be a frightening and challenging experience. Having an experienced Health Care Advocate on board to help field questions, manage appointments, provide insight, guidance, and give a little peace of mind can be invaluable. Whether you’re dealing with a new diagnosis yourself, or trying to manage an aging parent’s health, it’s nice to have someone with a wide range of experience to help guide you.

Dawn Harlick is a Registered Nurse with over 20 years of experience. Her Nursing career has ranged from helping patients manage their chronic health issues to helping them plan at end of life through Hospice and Palliative care guidance. Dawn is a current member of The Alliance of Professional Health Advocates.

Available services

  • Medication review
  • Medication education ( why am I taking this? Do I need all these medications?)
  • Patient Advocate and support during In Patient (hospital) stays
  • Collaboration with physicians and family, updates on medications, test results.
  • Assist with coordination of private hire care givers
  • Home safety assessment and recommendations on equipment
  • Dietary education (Diabetic diet, Low sodium, DASH diet)
  • Assess and refer for community resources (PT, OT, Home care etc)
  • Educate and assist with Advanced Directives, DPOA and understanding “Code status”
  • Interpreting your doctor’s orders and treatments
  • Provide education on hospice /palliative care

Research shows that increased compliance with out-patient or hospital discharge plans means fewer emergencies and hospitalizations.

Plans

Initial Comprehensive Assessment / Recommendations $100

1 hour in home visit recommended and 30 minute 2nd appointment for recommendations ( phone or in person).

During the assessment interview expect to be asked a lot of questions. I want to get the full picture of your circumstances, options, hopes and goals. To do so I will briefly review all important elements of your life and quickly focus on the areas of most concern to you. I will then create a comprehensive care plan. This care plan is your road map with resources and recommendations for the next steps. This assessment includes a review of Medical Concerns, Social, Home Environment & Physical Safety, and also collaboration with family members if requested. It is helpful to have a copy of your recent discharge paperwork or any medical records you feel important.

Ongoing Services/phone consultations/periodic reviews $60/hr.

Precautions are in place with each patient. I will wear a mask if requested. I am fully vaccinated and boosted.
